1. 福州大学国际交流生入学申请表（相关信息严格按护照填写），电脑填写，本人手签。链接见下方。

1.  Fuzhou University International Exchange Student Application Form (Information must be filled out strictly according to the passport). To be completed on computer and hand-signed by the applicant. (Link provided below).                          

2.本人护照首页复印件，有效期应覆盖交流结束后6个月。

2.  A copy of the passport information page. The passport's validity period should cover at least 6 months beyond the end of the exchange period.

3.近期彩色免冠证件照片6张（申请阶段电子版即可）。规格：二寸（3.5*5.3厘米），白色背景无边框，头部占照片尺寸的2/3。纸质版相片背面需注明姓名。特别注意：电子版照片尺寸不低于320*240像素，高宽比为4：3， 大小为100-500KB，必须发邮件至：faoadmission@fzu.edu.cn.邮件名称：学院名称+ 护照名字+接受学院。

3.  Six recent color passport-style photographs with a plain white background, no hat, and no border. Note: Electronic copies are sufficient at the application stage.

   Specifications: 2-inch size (3.5cm * 5.3cm), with the head occupying about 2/3 of the photo.

   For physical photos: Write your name on the back of each photo.

   For electronic photos (Important): Dimensions no less than 320*240 pixels, aspect ratio 4:3, file size between 100-500 KB. Must be emailed to: <faoadmission@fzu.edu.cn>. Email subject format: [College Name] + [Passport Name] + [Receiving College at FZU].

4. 派遣学校出具在学证明或交流协议，须加盖公章。

4.A Certificate of Enrollment or Exchange Agreement issued by the sending institution, bearing the official seal/stamp.

5.派遣学校出具的成绩单，须加盖公章

5.An official Academic Transcript issued by the sending institution, bearing the official seal/stamp.

6.外国人体格检查记录，6个月内有效。链接见下方。

6.Foreigner Physical Examination Form, valid within 6 months. (Link provided below).

7.无犯罪记录证明。通常应为提交申请之日前6 个月以内。链接见下方。

7.Certificate of No Criminal Record. Generally, it should be issued within 6 months prior to the application submission date. (Link provided below).

8.其他论文、获奖、科研成果、技能资格证书等文件以及参加文化体育交流活动的获证证明等。

8.Other supporting documents such as published papers, award certificates, proof of scientific research achievements, professional skill qualification certificates, and certificates obtained from participation in cultural or sports exchange activities, etc.

说明：在申报阶段，请将上述资料请清晰扫描后发至faoz@fzu.edu.cn。若学校审核通过（约需一个月时间），将通过邮件发送签证表与录取通知书。

Instructions: During the application stage, please scan the above documents clearly and send them to faoz@fzu.edu.cn. If the university's review is approved (which takes approximately one month), the visa form and admission notice will be sent via email.
